This form is for the purpose of adding free internet resources to the catalogue and the thematic listing. These resources are required to meet a set of selection criteria before being added. For paid subscriptions to electronic resources, please continue to contact Collections Services (Linda Ryan) 220-8509.

Please fill out section 1 if you want the resource added to the catalogue only.

Please fill out sections 1 and 2 if you want the resource added to both the catalogue and the thematic listing (aka the article index list)
